Required Forms
The list indicates the required forms for prospective new and returning Warhawk Crew rowers. Only new rowers are required to fill out the Swim Test Form. The Intent to Participate Form is required ONLY at the beginning of the rowing season.

Remember each rower needs to complete a physical exam before they join winter conditioning. The form for the physical is in the registration packets.
